You should bring a driver’s license or valid ID, insurance information, a list of medications you take, and any reports, X-rays, MRI scans, CT scans, or other relevant studies. If this is a first visit, you may print and fill out the general health questionnaire and the hand questionnaire ahead of time. Lastly, if you have a referral letter from another physician, you should bring this with you as well.
